Note: Chair refers to the home Department Chairperson of the student's primary academic plan.

Two concerns or a Level 2 was indicated on the Concern Form.

YES

Department Chair or Program Coordinator and Associate Dean are notified.

*Chair may elect to convene a committee to review Level 1 data and to discuss the concerns. Also, they can decide to immediately elevate the concern to Level 3.*

Chair or committee meets with candidate.

The Chair or Program Coordinator completes the Professional Dispositions – Action Plan Form (PDF fillable form) (signatures required). A copy of the form must be given to the candidate.

Will the candidate have to submit evidence relative to the Action Plan (e.g., reflective statement, paper)?

YES NO

After the due date for the candidate to submit evidence passes, the Chair or committee must hold a follow-up conference with the candidate.

Did the candidate successfully complete the Action Plan?

YES NO

The Chair or Program Coordinator completes the Tk20 Professional Dispositions Follow-Up Status Form and scans and uploads the signed Professional Dispositions – Action Plan Form.
