The Chair or Program Coordinator and Associate Dean conference with the candidate to discuss next steps.

The Chair or Program Coordinator completes the Tk20 Professional Dispositions Follow-Up Status Form and scans and uploads the signed Professional Dispositions – Action Plan Form. Status is indicated on the form. Proceedings move to LEVEL 4.

The Chair or Program Coordinator and Associate Dean conference with the candidate to outline the details of the Academic Support Plan and to complete the Professional Dispositions – Action Plan Form (signatures required). A copy of the form must be given to the candidate.

*If evidence is required, after the due date for the candidate to submit evidence passes, the Chair or Program Coordinator and Associate Dean must hold a follow-up conference with the candidate.*
